This may be a difficult subject for some to talk about but if you, someone close to you, a friend or relative is frail or very ill, it is a subject that you may need to raise sooner rather than later.

The needs and wishes of the individual, involvement, support and planning are all important areas ……… but what do you think as a patient, carer or relative?

Come and share your views and learn what Buckinghamshire Healthcare NHS Trust is doing to improve our care for patients who are at the end stage of life.
